THE SPASO TECHNIQUE


Ara fa un any posavem un video amb un cas pràctic de la. Ara us paso la referencia de un article que explica com.
THE SPASO TECHNIQUE : The patient is placed in the supine position; the affected arm is grasped around the wrist or distal forearm and gently lifted vertically, applying gentle traction. While maintaining vertical traction, the shoulder is slightly externally rotated (fig 1). A clunk is heard or felt when reduction is done. If the patient experiences discomfort, they tend to lift the shoulder up off the bed. If this occurs, stop further movement of the limb but maintain traction. The pain will usually subside quickly and the patient relaxes. After a few minutes of gentle traction, reduction will usually occur. If difficulty is experienced, it may be helpful to use one hand to palpate the head of humerus and gently push it to assist reduction, while maintaining traction with the other hand.

És una gran tècnica i a mi m'agrada molt. Ell no ho diu però en el moment que tracciones els desapareix bastant el dolor i això ajuda molt. De totes maneres crec que totes les maniobres són bones si el pacient es relaxa.

Yo no triunfo con esta técnica. Mi preferida es la de Arlt (fulcro en la axila), pero en nuestras urgencias no es fácil realizarla. El comentario de Annanota es acertado: conseguir la relajación del paciente.

Últimament davant la dificultat que suposa provar qualsevol mètode de reducció quan al pacient ja li han fet mal intentant alguna tècnica, m'ha resultat satisfactori (en 2 pacients a qui les últimes vegades els havies hagut de sedar) col·locar-los en prono amb l'espatlla a 90º (cap al terra). És l'Spaso, però sense haver de fer cap esforç. Em sembla recordar que és la tècnica de Stimson (no tinc cap llibre aprop per confirmar-ho) i no els fa gens de mal. El problema és saber en quin moment l'has reduït perque no es noten ni tan sols el clec de la reducció, demostrant que el mètode és molt poc agressiu. Recordeu, si us plau, que la reducció us serà fàcil si el pacient està relaxat, parleu-li, calmeu-lo i NO LI FEU MAL, o us serà molt difícil curar-lo. No hi ha reducció difícil, sino pacient mal relaxat!!!!!
